All students who intend to play intercollegiate sports at Medaille College must have four documents on file in the athletic department before they may participate in athletically related activities.

Those documents include:

Please schedule an appointment with your doctor to have the physical performed and return the completed form to the Athletic Department. If you are unable to see your own doctor for the physical please contact the Athletic Office, physicals will be conducted at Medaille on specific dates for a fee.

In addition to a physical, each athlete must carry his or her own health insurance. For students who have no insurance and are no longer covered under their parents’ plan, health insurance may be purchased through the college. Learn more about Intercollegiate Sports Insurance Coverage ».

No student-athlete will be allowed to participate in any way until evidence of current insurance coverage, including a copy of both sides of the insurance card and a physical form are on file with the department of athletics.
